Sutter's Continuum Case Management Department offers special resources for Sutter hospital patients and those who need extended care after they leave the hospital. Sutter Case Managers work with physicians, patients and families to determine the best next steps for each patient, which may include referrals to skilled nursing or custodial care facilities or to community programs. Patients may also be enrolled in one of the ongoing management programs:
